Service Coordinator
The role
Families come to ComfortZone when a relative needs more care than they can arrange alone. A service coordinator is the person who stays with that family through the NHTD or TBI waiver: more care hours, the documentation, and daily support that does not drop off.
What the work is
- Talk with the family about who needs care, and what a day at home looks like now
- Sort out whether the path is the NHTD waiver or the TBI waiver, and which paperwork that program asks for
- Prepare and track that documentation until it is in
- Line up the care hours, then check that the daily support is actually happening
- Stay the person the family calls when the routine has to change
Who you are with
Families in New York, often arranging care for a relative. You also work with the people who provide the day-to-day support in the home, so the hours that were promised are the hours that happen.
