ComfortZone supports families in three programs. In each one, we help increase care hours, process the documentation, and provide daily support.
NHTD
The Nursing Home Transition and Diversion waiver is a New York Medicaid program for seniors and people with disabilities who need a nursing-home level of care and want to live in the community instead of an institution.
ComfortZone helps with the documentation that process requires, and with increasing the care hours your relative receives. Daily support stays in place so they are under reliable supervision.
Participation is approved by the Regional Resource Development Center, not by this website. The New York State Department of Health publishes the program rules and the application steps.

TBI
The Traumatic Brain Injury waiver is a New York Medicaid program for adults whose brain injury leaves them in need of a nursing-home level of care, and who want to live at home.
We help families prepare the paperwork and keep daily support going, so your relative stays under reliable supervision.
Holocaust Survivors
Holocaust survivors and the families who care for them can come to ComfortZone for help with documentation and with daily support.
We work to increase care hours and to keep a reliable watch over the routine, so you can have peace of mind and confidence.
